1. Definition of terms

• Personal information: any information that identifies you, such as your name, age, contact details or photo.
• Sensitive personal information: includes your health and dental records, treatment history, X-rays and similar medical information.
• Processing: anything we do with your information, including collecting, recording, storing, using, sharing and deleting it.
• Platforms: the ways you reach us, including this website, our Facebook page and Messenger, Instagram, phone, email and our clinic.

2. Our services

MDN Dental Studio provides general and esthetic dentistry, oral surgery and orthodontic care at our clinic in Cainta, Rizal. We process personal information when you inquire, book an appointment, receive treatment, pay for services, or contact us after your visit.

3. Information we collect

• Basic details: name, age, date of birth, sex, address, mobile number, email address and preferred contact method.
• Health and dental information: medical and dental history, allergies, medications, consultation notes, treatment plans, X-rays, intraoral and clinical photos, and consent forms.
• Appointment details: services requested, preferred dentist, dates, times and notes you send us.
• Payment details: billing records, receipts and payment method used. We do not store full card numbers.
• Visual records: CCTV footage recorded in the clinic's common areas.

4. How we collect it

• When you fill in our website booking form or message us on Messenger, Instagram, phone or email
• When you fill in patient forms and consent forms at the clinic
• During consultations, examinations, X-rays and treatment
• When you pay for services
• Through CCTV cameras while you are inside the clinic

5. Why we use it

• To schedule, confirm and remind you of appointments
• To assess your condition, plan and provide your dental care, and follow up after treatment
• To keep accurate patient records as required by law and professional standards
• To process payments and issue receipts
• To respond to your questions, requests and feedback
• To keep our clinic, patients and staff safe
• With your separate consent only: to use your treatment photos or reviews for education or marketing, and to send you promotions

6. Storage and security

We keep your information in secure physical files and password-protected systems. Only authorized dentists and staff who need it for your care or for clinic operations can access it. Our team is bound by confidentiality and trained on handling patient data. We use reasonable organizational, physical and technical safeguards to protect your information against loss, misuse and unauthorized access.

About this website: the booking form does not store your details on our website. When you tap "Book via Messenger," your request is sent to our clinic page through Facebook Messenger, and Meta's own privacy policy also applies to that conversation.

7. Sharing and disclosure

We do not sell your personal information. We share it only when needed, and only under confidentiality obligations, with:

• Dental laboratories that make crowns, bridges, veneers, dentures or appliances for you
• Specialists or other health professionals you are referred to
• Payment processors, banks and e-wallets when you pay
• Service providers that help us run the clinic, such as scheduling, messaging and record-keeping tools
• Government agencies or courts, when required by law

8. Retention

We keep your records only for as long as needed for your continuing care and for the periods required by law and professional regulations. When records are no longer needed, we dispose of them securely: paper records are shredded and digital records are permanently deleted.

9. Your rights

Under the Data Privacy Act, you have the right to:

• Be informed about how your information is collected and used
• Access your personal information
• Correct information that is inaccurate or incomplete
• Object to processing, and withdraw your consent at any time
• Request deletion or blocking of your information, subject to our legal duty to keep medical records

10. CCTV notice

We use CCTV cameras in the clinic's common areas, such as the entrance and reception, for the safety and security of patients, staff and property. Cameras are not placed where you expect privacy. Footage is viewed only by authorized personnel, is kept only for a limited time, and is then overwritten, unless it is needed for an investigation or required by law.
